Employment Forms and Documents

Once you have been offered and accepted employment by Indian River State College, the following seven forms must be completed in their entirety.

•  I-9 – pdf
  Required by federal law (Homeland Security) and should be completed on or before the first day of employment. Please present, in person, documents establishing both identity and employment eligibility. Examples are a Passport or Driver’s License AND original Social Security Card. Other acceptable documents are listed on the back of the form.
•  W-4 Form for Tax Withholding  
•  IRSC Loyalty Oath
  Required by Florida state law (this form will need to be notarized).
•  Reverse 911 Emergency Data Information Form
•  Florida Retirement System (FRS) Certification Form
•  Direct Deposit Form
•  Acceptable Use Policy

  This application addendum must be submitted in conjunction with the IRSC employment application to be considered for any IRSC adjunct faculty position vacancy.
